Welcome to The Dentist. We welcome children and adults from Delavan and the surrounding Walworth County area. Call 262-728-9330 to arrange a visit and tell us about your dental concerns.
Before your visit: Download the health history form below and bring your completed copy. Forms are also available at the office; please arrive 10–15 minutes early if you need to complete one. Bring your medication list and dental insurance information, if applicable. Ask our team about transferring prior dental records securely.
Insurance and costs: Coverage varies by plan and treatment. Contact us before your appointment with insurance and payment questions. Our team can explain the next steps for your situation. Existing patients can use Patient Login or Make Payment for their account.
